ZILLIAN HOLISTIC WELLNESS CLIENT AGREEMENT & POLICY CONTRACT

This Client Agreement (“Agreement”) is made between Zillian Holistic Wellness (“Provider”) and the undersigned client (“Client”). By scheduling and receiving services, the Client agrees to the following terms and conditions:

1. APPOINTMENT BOOKING & PAYMENT
  • A non-refundable deposit of $50 is required to book an appointment. This deposit will be applied toward the total service cost.
  • Full payment is due at the time of service.
  • Accepted payment methods: Cash, Credit/Debit Card.
  • Gift certificates and package sessions are non-refundable.
2. CANCELLATION & NO-SHOW POLICY
  • A 24-hour notice is required to cancel or reschedule an appointment.
  • Late cancellations (less than 24 hours’ notice) will result in the forfeiture of the deposit, and a new deposit will be required for future bookings.
  • No-shows (failure to attend a scheduled appointment) will result in the Client being charged 100% of the service fee.
  • Clients with two or more no-shows may be declined future services.
3. LATE ARRIVAL POLICY
  • Clients who arrive late will have their session shortened to accommodate the next scheduled appointment.
  • Excessive lateness may result in cancellation of the appointment, and the Client will be charged the full session fee.
4. REFUND POLICY
  • No refunds will be issued once a service has been provided.
  • Deposits and prepaid sessions are non-refundable.
  • If the Client is dissatisfied with a service, they must notify the Provider immediately to discuss a resolution.
5. PROFESSIONAL CONDUCT & BOUNDARIES
  • Zillian Holistic Wellness provides professional, therapeutic massage and holistic wellness services only.
  • Any inappropriate behavior (including but not limited to sexual advances, inappropriate comments, or misconduct) will result in the immediate termination of the session, full payment required, and a permanent ban from booking future services.
  • Disrespectful, aggressive, or threatening behavior will not be tolerated.
6. HEALTH & WELLNESS CONSIDERATIONS
  • The Client agrees to disclose any medical conditions, injuries, allergies, or contraindications before the session.
  • If the Client is feeling ill or experiencing symptoms of a contagious illness, they must reschedule at least 24 hours in advance.
  • The Provider reserves the right to decline service if the Client appears to be under the influence of drugs or alcohol.
7. MOBILE & IN-HOME MASSAGE POLICIES

For mobile services, the Client must:

  • Provide a clean, safe, and quiet space for the treatment.
  • Ensure pets are secured to prevent distractions.
  • Allow reasonable access for the Provider to set up necessary equipment.
  • If the environment is deemed unsafe or unsuitable, the appointment will be canceled, and the full session fee will be charged.
8. LIABILITY RELEASE
  • The Client acknowledges that massage therapy is not a substitute for medical treatment and that the Provider does not diagnose or prescribe medical conditions.
  • The Client releases Zillian Holistic Wellness and its employees/contractors from any liability related to pre-existing conditions, reactions to treatments, or any injuries resulting from undisclosed medical conditions.
  • The Client agrees to receive treatments at their own risk.
